Summary of Objective Implementation Status

HSD is currently working on expanding integrated systems of care to address gaps in services to the County’s most vulnerable in two ways:  (1) launching HSD’s 2023-2026 Strategic Planning process in which we have identified Strategic Priority Area #2 to Enhance Service Delivery systems in partnership with other County agencies and community partners; and (2) preparing to open a satellite office at the Sonoma Valley East County Services Center which will be co-located with Department of Health Services, WIC and Behavioral Health, Child Support Services and Board of Supervisors District 1, Office of Supervisor Susan Gorin.   During our December All Managers Meeting, HSD’s Executive Team gathered input from staff on ways to strengthen our service delivery and how to innovate, adapt and evolve to better meet the needs of the most vulnerable in our communities.  We will continue working with all staff to further define the Priorities, Goals, Strategies and Measurable Objectives in a final report and recommendations by the first quarter of 2024.
